Required Information

Our Form Creation service, provided by our MyTime Support™ Team, assists with editing or creating a form on your ImageCafé® or Website Builder site. Price includes 1 form, up to 15 fields on the form.

In order to complete the service, we must receive information on the following:

• A listing of fields to be included on the form, including the desired order for the fields.
• Indicatation of which fields are required vs. optional.
• Indicatation if any checking should occur against fields, or if field entry should be limited to specific data elements.
• Information regarding on which page the form will be integrated, or if a new page should be created for the form.
• The address to which form responses should be sent.

• We cannot update DotNetNuke, ColdFusion, FrontPage, ASP, or ASP.net based websites.
